Middle School Traveling Soccer Registration

Do you LOVE SOCCER? 

Play on the Middle School competitive soccer team!

Registration is for girls in grades 6, 7 and 8 and boys in grades 6, 7 and 8 this fall.  Mail application forms or attend the registration session on August 24^th from 6:00-7:30 pm at Dairy Land Pavillion. 

Fees for 2010 season –   $85 registration fee

The registration fee covers costs associated with operating the traveling soccer program.  Items include but are not limited to:  Coaches stipend, mandatory accident insurance, referee fees, uniform costs, and field and equipment costs. 

Please include / bring child’s –

·         Medical insurance information

·         Emergency contact information

·         Medical Release form.
